Nether warts are naturally occurring plants that can be found in a nether fortress.
Nether fortresses, obviously, are found in the
NETHER. Nether warts are used for brewing potions.
Potions can be created with a brewing stand.

A nether wart can be planted on soul sand, another block only found in the nether.

Nether warts will grow slowly, and will not be sped up with bone meal. A fully grown nether wart may drop 4 nether warts when harvested.
